What you'll do
- Assess and treat patients in home and community settings
- Develop and implement individualized treatment plans
- Deliver specialized community and integrated care services
- Support patients in living safely and independently
- Provide patient education and rehabilitation services
- Work collaboratively with interdisciplinary healthcare teams
- Maintain clinical documentation and records
- Deliver care across multiple community locations
What you'll need
- Valid physiotherapy license/registration
- Professional credentials in physiotherapy
- Ability to work in home and community settings
- Valid driver's license and reliable transportation (implied for home health)
